Notification of serious harm and accidents
Employers, principals and the self-employed must:
- Notify your nearest regional Department of Labour Office (by phone or fax) as soon as possible of occurrences of accidents or serious harm; and
- Provide The Department written notice (using the form below, or providing the same details) of the circumstances within 7 days.
